Abstract

Objectives

To compare serum testosterone response and symptom improvement in men with hypogonadism in response to treatment with clomiphene citrate (CC), human chorionic gonadotropin (hCG), or a combination of both therapies.

Patients and Methods

A total of 282 men with hypogonadism, wishing to preserve their fertility, were randomized to one of three treatment arms: CC 50 mg (n = 95); 5000 IU hCG injections twice weekly (n = 94); or a combination of both therapies (CC + hCG; n = 94). All participants had complete medical history and had undergone thorough physical examination, including body mass index (BMI) assessment. Laboratory tests included serum total testosterone and glycated haemoglobin (HbA1c) measurements. Quantitative Androgen Deficiency in the Aging Male (qADAM) questionnaire scores were also recorded. Morning samples of total serum testosterone levels were assessed at three time points: baseline, 1 and 3 months.

Results

Testosterone levels increased at 1 and 3 months in all three groups. The mean baseline testosterone level was 2.31 ± 0.66 nmol/L, BMI was 30.8 ± 6.2 kg/m2, and qADAM score was 20.5 ± 3.8. Testosterone levels increased in all groups at all time points, with a final mean value of 5.17 ± 1.77 nmol/L (223% increase) with no statistically significant difference among the groups. qADAM scores had increased in all groups at 1 month (CC group: 6.36; hCG group: 5.08; CC + hCG group: 7.26) and at 3 months (CC group: 12.73; hCG group: 11.82; CC + hCG group: 15.13) with a significant difference in intergroup analysis for the CC + hCG group compared with the other two groups (P < 0.01).

Conclusions

All three treatments were equally effective in restoring testosterone levels. Single-agent CC is simple, cheap and may be used as treatment for hypogonadism when maintenance of fertility is desired. This approach seems to be as effective as either hCG alone or a combination of hCG and CC.

Visio Bedroom Furniture Shapes

If you need to plan your next furniture rearrangement, this elevation-view, scaled IKEA KALLAX Visio shape might help you get the job done!

We've just been looking at changing our living room cabinetry so that we can hide away more bigger things than our trusty old BILLYs are capable of.

When it comes to rearranging furniture, Visio is great for arranging objects from above in a plan view. But if you are considering a bunch of bookshelves, closets, or cabinets, the elevation view is arguably more important. Since Visio allows you to work at-scale, both plan and elevation views are no problem at all.

My wife was browsing BESTÅ bookshelves, and told me that she could use the IKEA home planner thingy to layout the bookshelves. Of course my pride was immediately damaged, but it was restored when she complained that "how do I update Flash on my iPad?". Wait, IKEA is using Flash for their online tool? That's a bit embarrassing, and it's certainly not happening on the iPad! Anyway, my pride was restored; "No prob, honey, I'll whip up some Visio shapes, and we can lay this thing out in no time!"

We have a couple of KALLAX bookshelves around the home and office, so I thought I'd start with those. KALLAX is interesting, because it comes in umpteen combinations, but all of the "cells" are the same size. This is perfect for making a massive SmartShape.

After too-many hours, I came up with One Kallax Shape to Rule Them All. The shape below can be configured to be 1 x 1, 2 x 1, 3 x 1, 4 x 1, 4 x 2, 4 x 3, 4 x 4, and 5 x 5! I wasn't able to find available-from-IKEA configurations in 3 x 2, 3 x 3, however.

I'm not sure if they ever existed, or if they have been discontinued, or what. But since I built the actual article numbers into this shape, I didn't want to build something you couldn't buy.

Anyway, here's an example of what this shape can do, complete with snapped-in accessories!

Please note that I do not have any affiliation with IKEA, other than being one of the horde who have bought tons (literally?) of their products. There are lots of Visio licenses out there and LOTS of IKEA furniture out there, so I thought some sort of symbiosis might be in order.

Here's a quick overview of all the features:

  • Bookshelf can be configured via right-click context menu, action tag, or shape data fields.
  • Bookshelf automatically resizes and recolors, according to the model you pick.
  • Bookshelf article number (e.g.: 703.098.40) is also automatically set when you pick the model. I did occasionally find differing article numbers for the same configurations on different country-sites for IKEA. So don't trust the article numbers 100%, but perhaps they will help some of you to generate a shopping list (using Visio's built-in reporting, even?)
  • Bookshelves manage connection points at the center of each "cell" to help with snapping/gluing of accessory shapes.
  • Bookshelves have a context-menu item that will take you to a Google search for the currently selected model. I wanted to use Google so that you have a better chance of getting a search result for your country.
  • Accessory shapes can be snapped/glued to the bookshelf, and will move with the bookshelf, once glued.
  • Accessories have article numbers as shape data fields as well, but only for the white versions. I didn't build spreadsheets to track all of the variations for these shapes–that would have been too much work! However, you can type in a different article number if you change the color, and still want to generate a report.
  • The books can be randomly generated. Just double-click a books shape, or right-click to re-randomize the sizes of the books.
  • You can right-click the door to quickly reverse the opening direction.

Here's an example of using the Action Tag that pops up at the top-center of the bookshelf shape. If you don't like right-clicking, then the Action Tag is a bit more accessible:

Format painter can match the accessories to the bookshelf, but the Article Number shape data field won't be accurate for the accessories when you do that. Be sure NOT to order white accidentally!

You can glue shelves to each other as well. Just grab near the bottom-center of a shelf shape, and drag near the top-center of another. I hope you have high enough ceilings!

If this shape turns out to be a hit, I would like to add some other popular IKEA items. Which families would you like to see? I think elevation views would also be useful, although they are pretty much just properly-sized rectangles.

Oh, and remind me to tell you about how I used Excel to build a complicated system of drop-down lists and lookup tables to extract RGB colors and article numbers from product names!

Get the Height Right
If  you're planning the installation for a low-ceilinged room, insist on a flush-mount model (also called a "ceiling hugger") to ensure adequate head clearance.

For average-height ceilings, using the manufacturer-supplied hanging rod should do the trick. For higher ceilings, an extension rod will lower the fan to optimal position within the room, about eight or nine feet off the floor.

Available in lengths from 12 to 72 inches, extension rods are for sale online and at home centers. To make sure its finish matches that of the fan, source the rod from the same company you buy the fan from.

Size Your Fan to the Room
Blade spans range from 24 to 80 inches. The American Lighting Association suggests the following guidelines:

Room SizeFan Size

Up to 75 sq. ft.                                      36 inches or smaller

75 to 144 sq. ft.                                     36 to 42 inches

144 to 225 sq. ft.                                   44 to 50 inches

225 to 400 sq. ft.                                  50 to 54 inches

For very large rooms with high ceilings, fans with 60- to 80-inch blade spans are available. Large-sized fans are as much about scale as function in meeting the requirements of a large space.

Place Your Fan Properly
Ceiling fans don't actually lower room temperatures; they cool by creating a breeze. Install them in places where you spend the most time. Good spots are over the bed or above family-room or kitchen seating.

Consider Control Options
Do you want to control the fan from a wall switch, a remote, or a good old-fashioned pull chain? You may not have a choice. Mode of operation depends on the fan that you choose. Tastes vary, but there is certainly something to be said for the convenience of a remote that enables you to change fan speed (or ceiling-fan light fixture settings) effortlessly.

Note: It's a good idea to keep the ceiling fan on a separate circuit from lighting. If both are on the same circuit, you won't be able to run your fan when the lights are off (especially important in the bedroom).

Shop Online
The universe of ceiling fans is amazingly big! Don't settle for the limited selection to be found in brick-and-mortar stores, especially if you need a fan with small blades or a model suited for cathedral or vaulted ceilings.

My favorite Web site for ceiling fans is Hansen Wholesale. Think of it as a sort of Consumer Reports. For each product it sells, Hansen offers information on motor quality, CFM (air movement measured in cubic feet per minute) output, energy efficiency and other pertinent data.

The ratings graphs show the average rating in each category, so you know immediately how the fan you're considering stacks up. Hansen also includes an excellent ceiling-fan size calculator.
